College Application / Scholarship Information Notebook will include:

1. One or two pages for each college you are considering for entrance and scholarship application.
2. The information that must be included on each page:

1.  The name and address of the school

2.  The phone number of the school to include the admissions department and 1-800 number.

3.  The web address of the school.

4.  The email address of the admissions department, representative.

5.  The deadline date for admissions application. (Slides, transcript, letters of recommendation, essay)

6.  The deadline date for scholarship application.  Is a separate application necessary?  Are separate slides necessary?

7.  How many slides for application / scholarship?  Are slides or digital images required?

8.  Information needed on each slide.

9.  How many letters of recommendation, from who?

10. School tuition for full time undergraduate day student.

11. Approximate cost of room, board, transportation, etc.

12. Is a personal interview necessary?  Deadline. 

13. Application Fee amount.

14.  Financial Aid Form Deadline.
